After submitting your entry, you will be taken to another screen showing your group sheet submission. Your group sheet will automatically be mailed to me, & one will come to you. You may send this one to anyone else who might be interested in this information for use on their web sites.

• DO NOT include LIVING PERSONS  on these charts (they will not be added).
• Family groupsheets should include MORE THAN ONE person.
• Put SURNAMES in capital letters.
• If there is more than one marriage, please fill out another group sheet & put #2 after husband's name.
• List all women under their maiden names, if unknown, then, please list as such.
• Add additional information in the area for documentation, below the children.
• ALTERNATIVES to this electronic form:
  *COPY AND PASTE this alternate email form  into an E-MAIL and fill in the blanks.
  *Try a method using Legacy software.
  These are the only 3 formats we can accept.
